The Elms Outpatient Services was developed to provide support to those patients who may require physical, occupational or speech therapy to achieve their optimum level of functional independence within their home and the community but who do not require an in-patient stay.
Conditions commonly treated in an outpatient setting include, but are not limited to:
In our outpatient program, we utilize a team approach in developing an individualized plan tailored to your unique needs. We will listen to you to learn how your condition affects your life. We want to know what your personal goals are – whether it be to speak clearly again, to work in your garden or to get back out on the golf course. We want to work with you to design a program that sets attainable goals that you feel good about.
The outpatient program can serve to aid an individual who suffers an injury or medical condition whose physician feels they would benefit from treatment or in some cases, for a continuation of therapy after a patient has returned home following an inpatient stay. If you have been with us before, you will enjoy the added benefit of working with the same therapists who assisted you during your inpatient stay.
Our objective is to aid the patient in the transition from an acute injury, post-surgical procedure or musculoskeletal dysfunction in achieving a return to the activities of daily living and enabling them to function effectively at a community level.
We offer all of the modalities available to our inpatient clients, including
These adjunct therapies, along with neuromuscular re-education, weight training and both active and passive range of motion exercises are combined to create a program that works for you.
