What is the difference between Long Term Care and Subacute Care?

Long Term Care provides 24 hour nursing care to residents who are medically stable but who are unable to care for themselves or by a loved one. Subacute care is for patients who have had a recent hospitalization and require a short stay in a facility where they can receive close medical monitoring and/or physical, occupational and speech therapy after which they return to their prior living arrangements.
