Admission Process

Subacute Rehabilitation and Nursing (Short Term)

Admission to the Short Term Unit generally follows an inpatient acute hospital stay.

Medicare guidelines require that a patient have a three midnight hospital stay in order to be eligible for Part A benefits in a skilled nursing facility.

The Elms has a nurse case manager who visits area hospitals daily to conduct on site clinical reviews of potential patients to determine appropriateness of placement.

In the event our case manager is unable to review your hospital chart prior to your scheduled discharge date, or you are hospitalized at a facility outside our travel distance for an onsite review, The Elms Admission Director will work closely with the hospital discharge planner to have your medical information sent to us directly for review by our facility team.
